What a pro does

Slabs are less common in New York than basements, but they're far from rare. Ranch homes built after the war, additions and attached garages poured on grade, and a good number of newer builds around the Hudson Valley and the western suburbs all sit on concrete. Radiant floor heating adds to the list, since those are water lines buried in the slab by design. Locating is most of the work. The plumber isolates the suspect line, pressurises it and listens across the floor with acoustic gear, and on a hot or heating line a thermal scan often shows the track of escaping water before anything else does. Where a heating loop is involved the system pressure gets watched over time, because a loop that needs topping up regularly is losing water somewhere. Once the position is known the options get laid out honestly. A single break in sound pipe can be opened and repaired locally. A line that's failed before, or a heating loop that's had multiple leaks, is usually better abandoned and replaced on a new route than chased through the floor a second time.

On a floor with radiant heat you often can't by feel alone, which is exactly why pressure is the better test. A heating loop that holds its pressure with the system off is intact, and one that drops steadily is losing water somewhere under the concrete.
A cold-water line escaping under a slab tends to show as a persistently damp or cool patch rather than a warm one, and near an unheated garage or an outside edge it can make a floor genuinely cold. Either temperature difference is worth investigating rather than living with.
It often is where the buried pipe has already failed once. A leak is usually a symptom of the whole run's condition rather than bad luck at one point, so opening the floor again in two years is a real risk. Running a new line through a wall or overhead leaves the slab alone.
It depends on the policy, and many treat the cost of getting to the leak differently from the cost of the resulting damage. Ask about both parts specifically, and contact your insurance provider before any concrete gets opened rather than after.
